[Iccrg] open issues draft

In Monday's ICCRG meeting, we mentioned the goal of finishing
up the document on open issues in internet congestion control
in the next several months.  Dimitri has prepared a slide
package on a small number of topics that the RG may want to
discuss.  His slides are here:

I believe the new topics that are described in the draft update
are:

- flow startup
- multi-layer interaction
- multi-path CC

There are also a few of the older topics that he flagged as
wishing to see further feedback from the RG on.

The tentative plan is to do a revision of the document by the
end of December, and between December and March hold an RG
review of it for moving it forward.
